The school will apparently be working with a new medical facility for 2016-17, so any forms and instructions from last season are not to be used until things are sorted out. Stay tuned for further updates and check the "Student-Athletes" link on the SLS site occasionally to see if any updates have been made to the "Medical Eligibility/Insurance" link. It's hidden for now until everything is determined. Thanks.
Found out that the university needs to acquire a new medical director to review all the forms mentioned in the earlier post. The timeline for this may take several weeks. Any physical done over the summer HAS to be on the correct form and done by a medical doctor, so until the forms are released, it's not possible to get a physical done that would meet the requirements. At some point in the future, it appears that there probably won't be an option to even get a physical done ahead of time and that all of them would have to take place on campus. Stay tuned...
The time frame scheduled for bowlers to be cleared by Athletic Training is currently 4-6 p.m. on Monday, August 22. This is also the session for bowlers to obtain a physical is needed.
EVERYONE wishing to practice, tryout, and compete has to be cleared in person prior to doing so! Plan accordingly.
It's official, EVERYONE will have to get their physical on-campus this fall. The session set aside for bowling is Monday, August 22 from 4-6 p.m. You will just need to show up with your current insurance card. It's mandatory to get this done before you are allowed to practice or tryout, so make this a schedule priority in you life.
FYI, a new software system called Vivature is being utilized, so be prepared to deal with that as well.
As a result of this, we can't have one of our meetings in the afternoon on Monday, the 22nd, so I'm trying to reserve some time in the afternoon on Tuesday, August 23rd. We'll still have the meeting opportunity during the evening on the 22nd at this point.
OK, it looks like there might be some walking back of some info. I received updated information regarding medical/insurance eligibility. It's all listed under the Student-Athlete menu on the Student Life Sports website, then select Student-Athlete Medical/Insurance Eligibility and read EVERYTHING on the page, including all links.
That should get you as up to speed as possible.
Once again, ALL new incoming and returning bowlers must be cleared IN PERSON by Athletic Training before practicing, or trying out. The time designated for bowlers to do this is Monday, August 22 from 4p to 6p. This is also the time frame available for anyone needing a physical.
Ask questions now, don't wait until school starts! Thanks.
If you haven't obtained a physical using the official form for this year by now, you MUST get your physical done on campus. All physicals in future seasons will have to be done on campus moving forward.
IMPORTANT: EVERYONE PLANNING TO BE ACTIVE IN THE PROGRAM AT ANY POINT DURING THE YEAR AND/OR WHO WILL BE BOWLING TRYOUTS MUST MEET WITH ATHLETIC TRAINING BETWEEN 4P AND 6P ON MONDAY, AUGUST 22 AT HYLAND ARENA. BRING YOUR CURRENT INSURANCE CARD WITH YOU AND FOLLOW THE SIGNS. THIS IS EVERYONE, NOT JUST THOSE NEEDING A PHYSICAL.
IT'S IMPORTANT TO READ ALL THE MEDICAL/INSURANCE INFORMATION ON THE SLS WEBSITE AND SET UP YOUR NEW VIVATURE ACCOUNT BEFORE THIS DATE! YOU SHOULD HAVE RECEIVED A NEW LOG-IN AND PASSWORD FOR THE NEW PORTAL THROUGH YOUR LIONMAIL ACCOUNT ON JULY 15. FAILURE TO GET THIS INFORMATION LOADED WILL MEAN A MUCH LONGER AMOUNT OF TIME THAT YOU'LL HAVE TO SPEND WITH AT ON MONDAY. IF YOU DID NOT RECEIVE THE LOG-IN INFO EMAIL ON JULY 15, PLEASE LET ME KNOW ASAP.
IF YOU HAVE QUESTIONS, ASK NOW!!! DO NOT WAIT UNTIL 6PM ON AUGUST 22.
NOTE: IF YOUR SCHEDULE DOES NOT ALLOW YOU TO GET TO HYLAND BETWEEN 4P AND 6P ON AUGUST 22, PLEASE VISIT HYLAND THE SAME DAY AT SOME POINT AFTER 11AM THAT MONDAY.
So, just a reminder that the medical insurance that you have MUST cover athletics in the state of Missouri. The secondary insurance that the school carries for injuries during athletic competition only kicks in AFTER the primary insurance is maxed out, and doesn't kick in at all if the policy doesn't cover athletics in Missouri. Might want to double-check...
