Please read all posts related to the upcoming 2013-14 season entirely in preparation for the start of the year.If you have any questions about anything, please let me know ASAP. Dont wait until the last minute.
MEETING
The team organizational meeting will be Monday, August 26th in the Leadership Room, located on the 4th floor of the Spellman Center from 4:15-6:15 p.m. This meeting is mandatory unless you are excused ahead of time for a really good reason and its scheduled so you can still attend even if you have class earlier in the afternoon. We will go over a wide variety of topics at this meeting and complete much of the paperwork necessary to compete in collegiate tournaments and prepare for try-outs and the beginning of the season in general, so PLEASE BRING SOMETHING TO WRITE WITH! IF YOU ASK ME OR RANDY FOR A PEN AT THE MEETING, I'LL DEDUCT 25 PINS FROM YOUR FIRST GAME OF TRY-OUTS.
UNIVERSITY-MEDICAL PAPERWORK
Here is a check list of many of things needed prior to the start of the try-out tournament that will begin on Saturday, September 14, 2013.The following must be done separately from our meeting. The medical eligibility items must be done prior to practicing or tryouts.
STUDENT-ATHLETE HANDBOOK ACKNOWLEDGEMENT FORM CHAMPIONS of CHARACTER PLEDGE LINDENWOOD STUDENT-ATHLETE DRUG TESTING POLICY
The new Student Life Sports Handbook for the 2013-14 season will be available soon. It outlines student-athlete guidelines and provides additional information about athletic facilities on campus. You will be required by the University to attend one of two meetings to review this Handbook and sign an acknowledgement form indicating that you have read and understood all that is outlined in it. You will also have to sign a Champions of Character Pledge form and a Lindenwood Student-Athlete Drug Testing Policy form. These three forms are required to be eligible for athletic gifts at the end of the year.Several either didnt sign it or turned it in past the deadline, so they didnt receive their gifts in past seasons.Stay tuned for information on the two meeting dates.
2013-14 STUDENT-ATHLETE MEDICAL ELIGIBILITY Prior to participation in any organized training, tryouts or competition, all Lindenwood University student-athletes must have a complete folder on file in the Athletic Training Department.Like last year, the SportsWare Online data entry system will be used.Specific step-by-step instructions for accessing and utilizing the system will be emailed to you (generally using your LU email address I believe).I have a copy of the letter that should have been sent and I will send it to your Lindenwood email by July 8, so please contact me if you didnt receive it either from the athletic training department or me by then.
Each athlete will also meet with a member of the AT staff to review all forms and information submitted.
The meetings for all bowling athletes have been scheduled for: Monday, August 26; All Day (this time frame may be narrowed) (Physicals, Ortho. SCAT2, File Review)Hyland Performance Arena
Here is a list of required information: Physical Examination.MUST use the LU Examination form (link to form below or find on Sportsware)http://www.lindenwoodlionssls.com/documents/2012/9/6/NCAA_Athletics_Physical_Form.pdf?tab=physicalexaminationform New student-athletes conducted within the past six months Returning student-athletes conducted within the past year I would STRONGLY advise having a physical done BEFORE returning to school, otherwise expect to spend a LONG time at Hyland Arena on the above date.
Sickle Cell Anemia Testing (can be completed prior to arriving or declined with signed waiver)
Concussion Fact Sheet (located on SportsWare-must be signed and submitted)
Emergency Contact Information
Primary Insurance Information Provide copy of insurance card (must be for ENTIRE year and submitted thru SportsWare)
Student-Athlete Waiver
I would strongly suggest reviewing the Student-Athlete Drug Testing Policy as well, available on SportsWare.
The following can be completed at the Bowling Organizational Meeting
YOU MUST HAVE YOUR MOST RECENT USBC MEMBERSHIP CARD or at least the number in order for us to sanction you for collegiate bowling. You can usually look this up on-line at bowl.com if you cant find your card.
ELIGIBILITY FORM Everyone eligible for competition this fall will need to sign an eligibility form so grades can be tracked throughout the semester. If you dont sign the form, you dont bowl or get sanctioned. We will not hunt you down!
Remember that grades are tracked at the four-week mark and at mid-terms in addition to the end of each semester and J-term. You must maintain minimal grade standards at mid-terms and after each semester and J-term to remain eligible to compete in tournaments.Additionally, the USBC-Collegiate requires the completion of 24 credit hours your first year of competition in order to be eligible for your second year, 48 hours prior to your third year and 72 years prior to your fourth year.
Slightly different is the University rule that requires you to complete 24 hours of credits in the time frame since the BEGINNING of your previous two semesters prior to competing in any semester or J-term.This one is a bit confusing!
BOWLER INFORMATION FORM-This is our own form that provides us with your basic contact information, shirt sizes, the type of bowling sanctioning you want for the year, as well as some other stuff. We will have this available at the organizational meeting.
BOWLER BIO FORM-This is our own form that asks for information that will be used publically on the Lindenwood athletics web site in conjunction with your listing on the bowling roster.
OFF CAMPUS PRACTICE RELEASE FORM-This form is required by the University because we practice off campus.
