Preparation for 2012-13 Season- University/Medical Paperwork


Please read all posts related to the upcoming 2012-13 season entirely in preparation for the start of the year.

UNIVERSITY-MEDICAL PAPERWORK

Here is a check list of many of things needed prior to the start of the try-out tournament that will begin on Saturday, September 1, 2012.  The following must be done separately from our meeting and prior to practicing or tryouts.

STUDENT-ATHLETE HANDBOOK ACKNOWLEDGEMENT FORM-The new Student Life Sports Handbook for the 2012-13 season will be available soon. It outlines student-athlete guidelines and provides additional information about athletic facilities on campus. You will be required by the University to attend one of two meetings to review this Handbook and sign an acknowledgement form indicating that you have read and understood all that is outlined in it. This form is required to be eligible for athletic gifts at the end of the year.  Several either didnt sign it or turned it in past the deadline last season and did not receive their athletic gift for the year.

2012-13 STUDENT-ATHLETE MEDICAL ELIGIBILITY
Prior to participation in any organized training, tryouts or competition, all Lindenwood University student-athletes must have a complete folder on file in the Athletic Training Department.  Like last year, the SportsWare Online data entry system will be used.  Specific step-by-step instructions for accessing and utilizing the system will be emailed to you (generally using your LU email address I believe).  Each athlete will also meet with a member of the AT staff to review all forms and information submitted.

The meetings for all bowling athletes have been scheduled for:
Monday, August 27; 12pm to 5pm  (Physicals, Ortho. SCAT2, File Review)       Hyland Performance Arena

Here is a list of required information:
Physical Examination.  MUST use the LU Examination form
            New student-athletes conducted within the past six months
            Returning student-athletes conducted within the past year
            I would STRONGLY advise having a physical done BEFORE returning to school, otherwise expect to spend a LONG    
            time at Hyland Arena on the above date.

Sickle Cell Anemia Testing (can be completed prior to arriving or declined with signed waiver)

Concussion Fact Sheet (located on SportsWare-must be signed and submitted)

Emergency Contact Information

Primary Insurance Information
                Provide copy of insurance card
(must be for ENTIRE year and submitted thru SportsWare)

Student-Athlete Waiver

I would strongly suggest reviewing the Student-Athlete Drug Testing Policy as well, available on SportsWare.

Additional instructions for Sportsware online.  IF YOU HAVE NOT RECEIVED AN EMAIL OR LETTER WITH INSTRUCTIONS FOR SPORTSWARE AND MEDICAL REQUIREMENTS NEEDED BEFORE PRACTICING OR TRYOUTS, PLEASE EMAIL ME ASAP AND I'LL SEND YOU THE INFO.  Thanks, Phil

Based on what I have been given in regards for new Sportware directions, the only things that need to be changed are the following:

 

1). The directions for logging in are the same as previous

2). Once logged in, athletes will see in the middle of the home page a number regarding how many forms are left incomplete

3). Click that number to advance to the forms page

4). Print off and complete all the forms

5). Back on the home page there is a tab labeled My Info select this tab

6). There are several tabs to choose from here: the athlete is responsible for filling out the General, Address, Emergency, Insurance, and Medical tabs.

7). After all of the tabs have been completed, click the save tab

Annual Physicals

In the past, after arriving on campus, many students have had their annual examination performed by the nurse practitioner in the student health center or the local chiropractic clinic.  However, these are no longer usable options.  The only individuals allowed to complete the form are medical physicians.  Physician assistants and nurse practitioners can only complete the examination under the supervision of a medical physician.  We have scheduled dates for all teams to have an opportunity to have their physical examinations performed on campus for $15 charge.  Bowling is Monday, August 27th from 12noon to 5pm.  This is also when ALL bowlers need to meet with an athletic trainer to review paperwork.

Students are still allowed to come to school with a completed physical (and I would encourage this!), but it needs to be completed by a medical physician and they need to use the NCAA (Lindenwood) form available on the athletics website
