Eligibility forms so grades can be checked and verified, due every semester.

List the number of the year you're bowling collegiately, print your name and SS # and sign.

No sign, no bowl...

Those not here for J-term will have to sign the form AS SOON AS THEY RETURN

yeah we have a good amount of people bowling and not bowling j term.. as long as everyone fills it out that's all that matters
