Medical Clearance 2015 (mandatory for ALL bowlers)


EVERY BOWLER WILL NEED TO MEET WITH CIARA DARGAHI BETWEEN 4-6PM ON TUESDAY, AUGUST 25 TO GET ALL MEDICAL/INSURANCE PAPERWORK CONFIRMED.  THIS IS ALSO THE TIME ASSIGNED TO BOWLERS WHO NEED TO GET A PHYSICAL DONE ON CAMPUS.  At this point, the location will most likely be Hyland Arena.  MAKE PLANS TO MEET WITH CIARA ON THAT DATE AND TIME.  OFFICIALLY, NO ONE IS SUPPOSED TO BOWL TRYOUTS OR COMPETE UNTIL SIGNED OFF BY ATHLETIC TRAINING.

Quick update...

If the student-athlete reports to campus with all the necessary documents completed (including their personal information on SportsWare Online), as well as a copy of their insurance card, it will greatly increase our efficiency throughout the medical eligibility process.  This information will be collected during your teams assigned medical screening date from 4-6pm on Tuesday, August 25.  

Also, as a reminder, the Primary Insurance Acknowledgement Form must be signed by the policy holder, not the student-athlete (unless the student carries his/her own policy).  This caused some delays this past year since we had to wait on several parent signatures.

Furthermore, while we will have physicals available on campus for a $15 fee, student-athletes are encouraged to have their physical examination conducted by their family physician (if they have one).  Typically, the family physician has a better knowledge of the student-athletes personal and family medical history and can often be more thorough when completing this examination.   

Lastly, the athletic training department has made a recent change to our physical examination requirements, and the following points highlight this change: 

(1)    Beginning in the 2015-2016 school year, returning student-athletes to Lindenwood University will only need to have a physical examination completed every other year. 

(2)    Returning student-athletes who submitted a new physical examination form during this past school year will need to complete a medical questionnaire and have some basic stats recorded (i.e. blood pressure, height, weight, etc).  These forms will be provided at the teams assigned medical screening day.

a.       Pending a review of this information, student-athletes may still be required to see a physician for further evaluation prior to being cleared for participation.

b.    If a returning student-athlete submits a completed physical exam form completed within the last calendar year, it can replace the medical questionnaire requirement listed above. 

(3)    All new and transfer students will still need to submit a current (within the last six months) physical exam prior to being cleared.

I JUST received this update from the Athletic Training department as of 8/18/15...

We have had to make a slight amendment to the medical check in schedule previously sent out.  All members of your team will still arrive on August 25th at 4:00pm to Hyland arena in order to complete their medical check in process as previously scheduled.

If any of your athletes need to complete a physical on campus, they are to arrive to Hyland arena on August 26th at 3:00pm to complete this portion of their medical check in process.  As long as they have provided all necessary paperwork, this will complete their medical check in process.

***Depending on the results of the medical questionnaire, returning athletes may still be required to have an updated physical.  These decisions will be made by the athletic training staff, and the athlete in question will not be allowed to participate until appropriate medical clearance is received.  In situations such as this, physicals may be completed in the Student Health Center, or off campus at a location chosen by the athlete***

Again, we suggest that all incoming freshman and transfer students complete a physical prior to arriving to campus.  However, for those in need, there will be a physician campus at the time listed above.

Anyone that wasn't able to make the medical "sign-off" meeting on Tuesday at the required time for bowling or if you need to still submit additional paperwork, you may do so by contacting Ciara Dargahi at cdargahi@lindenwood.edu.

 

I will receive a list of those who have been cleared by Athletic Training to practice, tryout, and compete.

This is a list of those that need to complete the medical/insurance clearance process as of 9/7/15!  Please contact Ciara Dargahi at cdargahi@lindenwood.edu and meet with her as soon as possible.
