The athletic trainer that is handling all of the medical/insurance eligibility paperwork for bowling is Ciara Dargahi.

EVERYONE planning to be active (practice, tryouts, etc.) needs to meet with her briefly so she can verify all of your paperwork. She will attend during the physicals time for bowling (12n-2p 8/25 at the Field House) and she will also be available at the Leadership Room in Spellman IN BETWEEN THE TWO BOWLING TEAM ORGANIZATIONAL MEETING SESSIONS ON AUGUST 25. (~5:30-6:30pm)

No one is supposed to be practicing until she's signed off that you have everything submitted properly.

If you have ANY questions about medical/insurance eligibility and paperwork, contact her directly BEFORE August 25.

Maybe something to take notes on, as well.

Be prepared to provide your current USBC Sanction Number and the type of membership you would want: Junior or Adult

We will be able to collect any tryout and jersey (shirt) fees at the meeting, as long as they're cash payments or checks written to St. Charles Lanes. Credit Card payments can be made at the bowling center, but only after Monday, August 25.

If you could bring a few current grey and jersey team shirts, so the new players will have some reference for sizing purposes, that would be helpful.

Remember, one of the two meetings is mandatory unless you have a REALLY good reason and you notify me in advance.
