MEDICAL/INSURANCE ELIGIBILITY

All members of the Lindenwood University bowling team are considered athletes and must meet a full list of requirements medically and from an insurance standpoint.  All of the requirements for new incoming student-athletes and returning student-athletes are listed on the Lindenwood University Student Life Sports website.

Please visit:  www.lindenwoodlionssls.com

Then click Student-Athletes in the upper RIGHT side of the home page.

On the drop-down menu, select Student-Athlete Medical Eligibility/Insurance"

You will be sent to a landing page where you can select the link that suits your situation.

ALL OF THE LISTED PAPERWORK MUST BE COMPLETED AND TURNED IN PRIOR TO PRACTICING OR PARTICIPATING IN THE TRYOUT TOURNAMENT.

The on-campus physical date/time for bowling is:  Monday, August 25th from Noon to 2pm in Hyland.  It will cost at least $15.

Expect to spend the ENTIRE afternoon standing in line if you choose this option.

I WOULD HIGHLY ENCOURAGE EVERYONE TO TAKE CARE OF THIS REQUIREMENT BEFORE RETURNING TO SCHOOL.  See the link above to get the proper paperwork needed from your doctor.

FYI

The university updated its insurance policy as of August 1, 2014.  If you turned in all of your paperwork prior to this date, you need to revisit the link above, review the insurance policies, and complete the Primary Insurance Acknowledgement Statement form.

The athletic trainer that is handling all of the medical/insurance eligibility paperwork for bowling is Ciara Dargahi.

Her contact info is:  cdargahi@lindenwood.edu or314-443-8962.

EVERYONE planning to be active (practice, tryouts, etc.) needs to meet with her briefly so she can verify all of your paperwork.  She will attend during the physicals time for bowling (12n-2p 8/25 at the Field House) and she will also be available at the Leadership Room in Spellman IN BETWEEN THE TWO BOWLING TEAM ORGANIZATIONAL MEETING SESSIONS ON AUGUST 25. (~5:30-6:30pm)

No one is supposed to be practicing until she's signed off that you have everything submitted properly.

If you have ANY questions about medical/insurance eligibility and paperwork, contact her directly BEFORE August 25.

Anyone still needing a physical or needing to turn in medical/insurance paperwork can do so TODAY (8/26) at Hyland Arena between noon and 4pm.  It's another general session, so you'll just go from station to station as needed to obtain your physical and/or submit all of your paperwork. 

They can also answer any questions you have.  Our specific AT Ciara will be splitting time between Hyland and the Field House, so during this time frame, you do NOT need to search her down specifically.

As a reminder, everyone needs to meet with athletic training to get "signed off" that all of your medical/insurance paperwork is submitted.  You can do that at Hyland during one of their sessions, or contact Ciara directly at the info above. 

If you give your paperwork to me, I'll just give it to Athletic Training, but you'll still have to meet with them personally.

 

I will be receiving a list of those who are approved.  If you're not on that list, you can't practice or tryout.  Thanks!

If you still need to turn in paperwork for medical/insurance, you may do so at the Field House, room 116.  You don't need to find our specific athletic trainer.  I hope to have an updated clearance list in the other medical thread by 9/4/14.

I've had a conversation with Athletic Training and the main reason that many haven't been cleared is that you haven't physically met with Ciara to verify all of your paperwork (as indicated in earlier threads) and your Sportsware account.  So, even if you've turned in everything, unless you've met with her to verify this, you're still technically not cleared.  Those that aren't showing up on their list at all have yet to set up their Sportsware account online, so that still needs to be done. 

Her office hours are generally in the morning at Field House 116.  We may set up a separate date and time to execute these meetings, but in the mean time, be proactive and set up appointments with her to finalize everything.  Please get this done, especially if you've been procrastinating or you won't be able to compete!

Here is the best time to meet with Ciara at her office, Field House 116.

Monday: 9-10
Tuesday: 2-4
Wednesday: 10-12 and 1-3
